What constitutes a Force Account report?

What constitutes a Force Account report? (In other words can it be like a shopping list, listing the amount of items but without the dollar value)? Does it need to be priced?

ANSWER

Article 4.4.6 in the Volume I User Guide discusses Force Account reporting. Daily report formats are often established at the pre-job meeting.

Article 4.4.6.5 references Exhibit #7 as a sample Daily Force Account Report. This is on the MMCD website at https://www.mmcd.net/documents/forms-and-templates/

The Site Inspector or CA must sign off on the quantities submitted on the forms. Article 4.4.6.1 outlines the process if there is disagreement. Payment for disputed items can be resolved by the CA, or failing that, through the Dispute process. However, it is deemed important to have evidence of the quantities stipulated on the forms, for later reference.
